PATIENT ACCESS TO MEDICAL RECORDS

You may wish to access your medical records by applying for on-line services.

To arrange this, you can call in to the surgery, with appropriate identification (a recent utility bill with your current name and address, plus driving licence, passport or other official documentation) and you will be provided with a password. Please keep this safely and do not share it with anyone.

You can then register for the NHS app or Patient Access on line.

Parents can apply for on line access for their children, but we do advise that once the child is competent to understand how to manage their own access, that parental access is revoked for privacy.
